Activity Report
This form is designed to document information about your activities. Prior to your activity download it, print it and then use it to record all of the great statistics about your activity. Although filling out this form is optional, we ask that you complete it and return it to us via fax, e-mail or traditional mail (see below) so we can compile information about all of the great work you’re doing. |

CWM I.D. Badge
What would a fun, social activity like a cleanup be without the proper name tags- especially if you have a large assembly of volunteers or new additions to your group? Prior to your activity, download the I.D. Badge and print out enough copies for you and your volunteers. |

Underage Waiver
If you are an Environmental Director and under the age of 18, we require you to fill out an Underage Waiver prior to conducting your first activity. We only require you to complete ONE Waiver, not one before each cleanup/activity. Once you have completed your Waiver, send it to us via fax, e-mail or traditional mail as soon as possible (see below).
Also, we do not require persons under 18 who are volunteer participants in a cleanup/activity to complete a Waiver. As a Director, you may choose to ask your volunteer participants under 18 to complete the Underage Waiver for your own benefit, but these will be for your records only. We do not need copies.
